What the inspector wrote

An unannounced case management inspection was conducted in response to a unusual incident report today at 12:06pm by Licensing Program Analyst (LPA), Sydney Sims. LPA met with Licensee Pamela Lee, In response to an Unusual Incident Report received by the Department on 8/21/25, where a child (C1) sustained an injury that resulted in a bloody nose and scrapes to the nose The Licensee was interviewed on 09/05/25 at 12:21 pm and stated that on 08/04/25 child C1 was playing on the playground with a large toy car on the pavement and the child fell and hit C1's face on the ground and when C1 stood up C1 got a bloody nose. Licensee stated that first aid was given to C1 and C1 was taken to the doctor by C1's parent and ointment was give to C1 for some cuts on C1's nose. Licensee stated that proper supervision was being provided and the toy car was age appropriate. During today\342\200\231s inspection, the facility was toured and LPAs observed 6 children in care. Although child C1 did receive an injury while in the care of the facility it could not be determined that any regulation violation occurred. Exit interview conducted and report was reviewed with the Licensee Pamela Lee appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
