What the inspector wrote

On December 4, 2025, an unannounced case management inspection was conducted by Licensing Program Analyst Staicy Perry, in response to a change in facility Director. LPA Perry met with Tina Lee, Assistant Administrator and newly appointed Director Desiree Wong. Census was taken and LPA Perry observed a total of 37 children in care. Director Desiree Wong started on December 1, 2025, and LPA Perry was notified of the change on December 1, 2025 with supporting documents by Tina Lee, Assistant Administrator. LPA Perry received and reviewed all required documentation related to the facility’s transition of director responsibilities to Desiree Wong during inspection. LPA conducted a checklist review with the new director and Tina Lee, Assistant Administrator. LPA Perry completed and discussed a LIC 308 Designation of Responsibility form. The new director Desiree was informed that the administrative records will reflect the change in director once all documents are officially processed in the system. LPA has confirmed all documents have been submitted .LPA Perry did also provide resources on Healthy Schools Act. At this time, no deficiencies were cited. A Notice of Site Visit was issued and will remain posted at the facility for public viewing. LPA discussed and provided appeal rights with the facility and exit interview conducted with Tina Lee, Assistant Administrator.
