Child 2 proceeded to walk to obtain their personal belongings alone. Staff 5 encountered and witnessed Child 2 alone while gathering their belongings.

Of the 5 Staff interviewed, 2 Staff members were able to corroborate the allegation indicating they witnessed Staff 6 pull the arm of a child in a rough manner while in the outdoor play area.

This requirement is not met as evidenced by: Interviews were conducted with Staff 1 (S1) to Staff 5 (S5). Of the 5 Staff interviewed, 1 Staff member was able to corroborate the allegation indicating that they witnessed a Staff member not providing supervision to a child which led to a fall. LPA reviewed facility report (date 11/22) which documented the fall and that ice was applied to the injured area.
