    101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time.....This requirement is not met as evidenced by: Per Staff 2, Child 2 was told to gather their belonging in preparation for leaving the facility for the day. Child 2 proceeded to walk to obtain their personal belongings alone. Staff 5 encountered and witnessed Child 2 alone while gathering their belongings.

This requirement is not met as evidenced by: Interviews were conducted with Staff 1 (S1) to Staff 5 (S5). Of the 5 Staff interviewed, 2 Staff members were able to corroborate the allegation indicating they witnessed Staff 6 pull the arm of a child in a rough manner while in the outdoor play area.

    101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement is not met as evidenced by: Interviews were conducted with Staff 1 (S1) to Staff 5 (S5). Of the 5 Staff interviewed, 1 Staff member was able to corroborate the allegation indicating that they witnessed a Staff member not providing supervision to a child which led to a fall. LPA reviewed facility report (date 11/22) which documented the fall and that ice was applied to the injured area.

This requirement is not met as evidenced by Based on interviews , the licensee did not comply with the section cited above in that interviews disclosed a fly problem, flies have been observed to land on food and milk was not kept cool which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by Based on interviews , the licensee did not comply with the section cited above in that interviews disclosed staff being out of ratio during nap time, which poses/posed a potential health, safety or personal rights risk to persons in care.
