What Happened
The annual inspection found several recordkeeping gaps for children and staff, including a missing immunization record, missing nap-check documentation, and an incomplete personnel file for the assistant. The home setup also showed some access-control problems, with children able to reach the bathroom and gated driveway while other off-limit areas were secured.
Observations
- Child 2 did not have an immunization record available for review.
- Children 1 and 3 did not have 15-minute nap check verification available.
- The assistant was missing CPR/first aid, immunization, TB, and employee rights documentation.
