What Happened
Annual/random inspection identified critical gaps in child and staff record documentation, including missing infant sleep plans and immunization records.
Observations
- Three children missing Individual Infant Sleep Plan (LIC 9227)
- Child #2 and #3 missing immunization records
- Staff missing current mandated reporter certificates
- Staff missing records of immunization against measles, pertussis, and tuberculosis
- Poison storage cubby has no lock (inaccessible but should be locked)
