Staff Infant Ratio: 101416.5 (e) There shall be provision for overlap of staff for different shifts, so that “continuity” of care is assured.This requirement is not met as evidenced by:Based on 26 interviews with staff and parents and record reviews. On 11/03/22 between 7:15am to 9:30am, one staff with 7 infants.On 11/03/22 between 12:00pm – 2:30pm, one staff taking care of two classrooms with children not napping. Due to the divider between the infant and toddler classroom, 8 children on infant side and 8 children on toddler side with children not napping.On 11/08/22 at 11:32 am, the infant classroom had two staff and 11 infants; 4 months to 12 months years of age documented on the attendance log.On 12/05/22 at 6:52am to 8:15am, between 8:00am to 8:06am with Staff#1. At 8:15am there were 16 children ages 2,3, and 4 years old with Staff#1 until 8:15am when children were moved to another location-documented on the Face-To-Name Transition Sheet. On 12/06/22 at 6:30am until 8:00am, there were 14 children mixed, ages 2, 3 and 4 years old with Staff #3 until 8:00am and then left with staff #1 documented on the Face-To-Name Transition Sheet.On 12/19/22 at 8:30am, there were 15 children ages 2 and 3 years old with Staff #4. At 10:03am, Child #1, #2, and #3 were moved to the office to maintain ratio documented on the Face-To-Name Transition Sheet.On 12/19/22 at 8:30am to 8:35am, Staff#4 was with 6 children ages 2 and 3 years old. At 8:36am, 8 more children for a total of 14 children arrived until 9:54am documented on the Face-To-Name Transition Sheet.On 12/29/22 at 8:30am to 12:30pm, Staff #4 was with 15 children ages 2,3, and 6 years old.On 01/11/2023 between 7:41am to 7:55am, Staff#1 was with 19 children. Children departed at 8:00am with Staff #5 documented on the Face-to-Name Transition Sheet.On 01/31/2023 from 8:30am to 9:21am, Staff #4 was with 16 children documented on Face-to-Name Transition Sheet.On 1/31/2023 from 6:30am to after 8:30am, Staff #3 and Staff # unknown was with 11 children.On 1/31/2023 after 8:30am, Staff # 5 was with 16 children. Which poses an immediate Health, Safety, or Personal Rights Risk to children in care.

INFANT CARE ACTIVITIES
101430(a)(3)(EIf the infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible as required by 101430(a)(3)(E This requirement is not met as evidenced by: Based on LPA Interviews and Photos: During October and November 2022Child #1 and Child #2 were allowed to , sleep on the floor in an unsafe position on a donut shaped pillow on the floor in the infant room. Photo #1 depicts Child #1 an infant sleeping on the donut pillow with the head tilted forward. Photo #2 depicts child #2 asleep on donut pillow on the floor, sleeping on their face. The children were not transferred to the crib in a timely manner after falling asleep thus not following infant safe sleep regulations. Which poses an immediate Health, Safety, or Personal Rights Risk to children in care.
