Inspector’s narrative

What the inspector wrote

Page 2 Per staff, there are times when incidents occur quickly before they can be prevented. Per Staff, this most often happens when a staff is completing another task. Per parent interviews, three parents stated their children had recently been bitten. Parents stated they receive a written report stating the incidents occurred but do not receive more information. Per staff interviews, children are transitioned to the infant classroom at the end of the day. An incident occurred in which one child was not taken with the rest of the group. Per Staff, it was per the request of the parent and was a misunderstanding or mis-communication. Per staff, child was not intentionally separated from the group of children as a punishment. Per interviews conducted with three other parents, their children have not been treated differently than other children and they have not witnessed staff using any inappropriate forms of punishment. LPA interviewed staff regarding the third allegation. This was reported due to an incident in which a parent was told there may not be enough breakfast for their child although child was dropped off during the scheduled meal time. Per staff, they were advising parent of the schedule because the child had recently transitioned to the classroom. Per staff interviews, there is always enough food to be served to the children. If food runs out during a scheduled mealtime, they will still serve the children other food or look for more of that meal in the facility. LPA observed the kitchen stocked with food. Per parent interviews, if a child arrives after 8:40AM, the staff will ask if the child needs to be served breakfast. Based on information obtained during the investigation, the allegations were determined to be unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. No deficiencies were cited today. Exit interview was conducted with Director Ana Fragoso. The Notice of Site Visit (LIC 9213) and the Appeal Rights were given and explained to the Facility Representative. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
