What the inspector wrote

Licensing Program Analyst (LPA) Saul Valenzuela conducted an unannounced Case Management inspection due to two incidents that were reported to the Department on 12/11/2024 and 1/14/2025. LPA met with Principal Sharon Harris who guided LPA on a tour of the facility. Census was taken. On December 11th, 2024, one incident was self-reported to the Department via Email by the facility who reported a child sustained an injury during outdoor play time. On January 14th, 2025, one incident was self-reported to the Department via Email by the facility who reported that on two occasions children were left unsupervised by a staff. All reports were reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incidents reported to the Department. Regarding incident that occurred on 12/11/24 interviews were conducted, no disclosures were made. Regarding incident reported on 1/14/2025: On two occasions children were left unsupervised by staff #5. Staff interviews disclosed that additional staff were present when Staff #5 would leave the area without the children. According to Staff #2 and #3, children were supervised at all times and there was no gap in supervision. LPA discussed Title 22, Responsibility for Providing Care and Supervision with Principal. ----------------------------- -----------------------------------pg. 1 of 2 ------------------------------------------------------- At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days.
