What the inspector wrote

Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced Case Management- Incident inspection on this date to gather information due to 8 incident reports that were reported to the Department on 1/23/2026, 3/10/2026, 3/16/2026, 3/17/2026, 3/23/2026, 2 on 3/24/2026 and 3/25/2026. LPA met with Interim Principal Allison Speight who guided LPA on a tour of the facility- Per Principal there are 89 children enrolled. Census was taken. On 1/23/2026 the facility reported that staff witness a staff member violating child # 1 personal rights. On 3/10/2026 the facility reported that parent of child # 2 had concerns regarding child's # 2 diapering needs. On 3/25/2026 the facility reported an incident regarding child # 3 falling and requiring medical attention. On 3/16/2026 the facility reported an incident regarding child # 4 falling and requiring medical attention On 3/17/2026 the facility reported that parent of child # 5 disclosed their child's personal rights being violated by staff. On 3/24/2026 the facility reported that child # 6 fell hurting the back, and their head On 3/24/2026 the facility reported that they received communication from LAUSD Transportation that parent alleges that their child's (child # 7) personal rights were violated by staff.----- pg. 1 of 2 ------ On 3/25/2026 the facility reported that child # 8 fell requiring medical attention. During inspection LPA conducted interviews with staff and children regarding incidents that happened on 1/23/26, 3/10/26 and 3/25/2026. Per Principal Speight they did not have any information regarding incidents reported on 3/16/2026, 3/17/2026, 3/23/2026 and 2 on 3/24/2026 and will follow up with the principal that was covering on those dates. LPA discussed reporting requirements with facility representative on this date. LPA will come back on another date to conduct more interviews regarding the 8 incidents mentioned above. At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days.
