What the inspector wrote

On 4/2/2025 Licensing Program Analyst (LPA) Loyce Phillips arrived at above mentioned facility for the purpose of conducting unannounced Case management deficiencies visit. Upon arrival, LPA met with lead teacher, Crystal Cervantes. Director, Pamela Dale arrived at 9:15 am and LPA discussed the purpose of the visit. LPA observed 13 children present with 2 staff members. Description of the incident : On 9/20/2024, Staff 1 (S1) reported Child 1 (C1) was playing with a vanity mirror in the classroom, the mirror fell over and landed on child's finger. First aid was applied, an ouch report was completed, and parent was called right away. Parent called Director at approximately 1:30pm and stated she took her child to Urgent Care. Director did not notify CCLD regarding the incident that required medical attention. A Type B citation is issued for reporting requirements. An exit interview was conducted, a copy of this report, appeals rights and notice of site visit was provided to Director, Pamela Dale.
