What Happened
Required 3-year inspection identified significant compliance gaps: fire extinguishers not serviced, playground equipment damaged/unsafe, medications improperly stored and labeled, staff missing required immunizations and mandated reporter training, and lead testing not completed. Four Type B violations and two technical violations cited.
Observations
- Fire extinguishers not serviced within past 12 months (last service 1/24/2024)
- Caterpillar play structure has broken plastic creating hazard to children
- Playground rug torn and worn, poses risk of harm to children
- Medications stored on top of refrigerator in kitchen, not properly labeled per child
- Two of four teachers have no immunization records on file
- Two of four teachers lack TB test documentation
- Two of four teachers have only flu declinations without supporting documentation
- No teachers have current Mandated Reporter Certification on file
- Lead testing not completed per Written Directives PIN 21-21.1-CCP
