What the inspector wrote

On 2/26/2024 LPA conducted an unannounced Case Management Incident inspection and met with facility Site Supervisor Gloria Mancilla. LPA disclosed the purpose of the inspection and was granted entry into the facility by a facility. No children were present during the inspection. The purpose of the inspection was to follow up on a self reported unusual incident report (UIR) which occurred on 2/15/2024 at the facility. The Department was notified of the UIR on 2/16/2024. According to the UIR, some classrooms required repairs as a result of the extreme weather conditions. No Health or safety concerns are present. According to the UIR, the classroom roof is being repaired, therefore the facility was closed 2/15/24 and 2/16/24. During the inspection today, LPA interviewed staff member and completed a safety check of the facility. Roof repairs are being completed and anticipate to be completed by Monday 02/19/2024. Facility plans to reopen for child care starting Monday 2/19/2024. LPA completed a safety inspection of the 11 classrooms and play yard. Zero deficiencies were observed. During today's inspection, LPA obtained documents related to the UIR. No further investigation is required. Facility notified parents of facility closure dates 2/15/2024 and 2/17/2024 in writing , Learning Genie app, and verbally. An exit interview was conducted, a copy of this report, and notice of site visit were provided to the facility Site Supervisor.
