On 03/25/2022 at 4:25 p.m., Licensing Program Analysts (LPAs) Silva Garibyan and Antonio Almanza conducted an unannounced Case Management Visit at the Probationary Licensed Facility for the purpose of following up on the unusual incident that occurred at the facility on 03/21/2022. During today’s visit LPAs met with the Director, Gloria Avina, explained the purpose of the visit, and toured the facility. During today’s visit there are 2 adults providing care and supervision to 16 children in care. During the inspection, LPA's conducted interviews with facility staff involved and Child 1. LPAs also reviewed child’s record and were supplied with copies of supporting documents. During this visit, LPAs obtained a copy of the child's records. At this time, further investigation is needed. Exit interview conducted with director, Gloria Avina and a copy of this report was provided .
