What the inspector wrote

On 7/23/25, at 1:30PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. Upon arrival, LPA met with Administrator / Gabrielle Hovey, who guided LPA on a tour of the facility. There were 15 children with 2 staff observed present in the preschool program. LPA explained the purpose of today's visit is to follow-up on an alleged incident that was reported to Community Care Licensing (CCL) on 7/21/25. The alleged incident that occurred on 7/19/25, was reported to the Department on 7/21/25, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. During today's visit, LPA Katrdzhyan toured the physical plant (preschool room only) and conducted an interview of the Administrator. LPA also obtained copies of children's and staff rosters and other pertinent documentation. Based on the information gathered, this incident will need further investigation. There were no deficiencies cited during today’s inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with Gabrielle Hovey.
