What the inspector wrote

On 10/29/2025, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced Case Management – Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 10/20/2025. Upon arrival, LPA met with Administrator, Gabrielle Hovey , and explained the purpose of the visit. LPA observed 17 children in care, being supervised by 3 teachers/aides. According to the UIR, on 10/17/2025, Child 1 (C1) tried to climb up cubby shelve, C1 knocked off part of the shelf, it fell and hit C1's left ankle foot or leg. C1 had a pink mark on their leg. Staff called C1's parents around 6:30pm just as dad arrived. C1 ran to the dad with no issue. Dad was upset they did not call them when it happened. Staff told dad they were very busy. C1 did not go to the doctor right away. Per meeting held, C1 was evaluated by a doctor at a later time and parents confirmed that there were no injuries or broken bones. During today’s inspection, LPA toured the facility and conducted interviews. LPA obtained pictures of the preschool classroom and observed the cubby that had fallen over while C1 attempted to climb was secured to the wall along with other similar cubby. Both were secured to the wall. LPA observed, in the same classroom, a tall rolling shelf and advised Administrator to ensure it is also secured to the wall to enure safety. Administrator agreed to provide LPA with pictures once secured. At this time, no further investigation is needed. An exit interview was conducted with Administrator, Gabrielle Hovey. A copy of this report (LIC 809) and Notice of Site Visit were provided and must remain posted for 30 days.
