Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced Case Management-Other visit on 10/03/2025 at 10:45 AM. LPA identified self and met with Debbie Heim, Director, who guided LPA on a tour of the facility. There were 24 children and 9 staff present. The purpose of the visit was to obtain copies of documentation for Staff #10, a previous employee. There were no deficiencies cited during today’s visit. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. The Exit interview was conducted with Christina Debbie Heim, Director including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
