On 10/14/2024, Licensing Program Analyst (LPA) Andrew Alemoh met with licensee /director for the purpose of following up with the unusual incident that was reported to the Palmdale Regional Office on 10/10/2024. The facility also submitted a written Unusual Incident Report (LIC624) on the same day. LPA Andrew Alemoh toured the facility and observed 0 children present and 0 staff present at the time of this visit. Per director statements children attend the after school program along with the staff members. On 10/10/2024 the facility reported an Unusual Incident concerning child #1 was playing on the playground on the monkey bars. C1 was jumping off the steps when C1 fell face down on their left arm. Resulting in a broken left arm. The facility provided First Aid and contacted C1’s parents. During today Case Management Incident visit, LPA conducted interviews with the director, children, and other parties including the review of supportive documentation and records. LPA inspected the playground area and were the monkey bars are located. LPA obtained photographs of the play area equipment. A final determination has not been made and further follow up is needed. No citations are being issued on this date. A copy of this report was read, and provided to the licensee/director along with a copy of the appeal rights and notice of site visit.
