Staff S1 & S2 were not in close proximity, they did not have visual observation of C1 and did not witness C1 fall. S1 & S2 learned of the injury after C2 & C3 brought C1 to them which is a potential health, safety, and personal rights risk to the persons in care.

This requirement is not met as evidenced by: Based on interviews and records, it was determined that C1 was left unsupervised in the outside play yard of the facility for an approximately one minute.
