Incident investigation·April 14, 2026

On April 14, 2026, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office on April 6, 2026. LPA met with Jacqueline Major, facility director, and toured the facility together and observed no school aged- day care children and one staff member in active care. All adults 18 years and over have Criminal Record Clearance (DOJ) and Child Abuse Central Index Clearance and are associated with the facility. At 3:00 PM thirty-four (34) school aged children arrived and three staff. On April 6, 2026, the reported information involved a child (C1) who was playing on the playground and swinging on the bars then C1 fell into the wood chips. Staff think the right elbow was dislocated but did not witness the incident. Staff notified C1’s parent. When the parents arrived C1 were taken to the emergency room for further medical assessment. During this inspection, LPA conducted interviews with relevant parties involved and conducted a safety inspection of the playground and viewed surveillance footage of the incident. Based on interviews, further investigation is needed. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, was provided to Jacqueline Major, Director at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting may result in a $100 civil penalty.
