(d)...a report shall be made to the Department by telephone or fax…[and] a written report containing the information…shall be submitted to the Department...(1) Events reported shall include...(C) Any unusual incident...that threatens the...health or safety of any child. This requirement is not met as evidenced by no LIC 624 submitted to the Department. Based on interviews conducted, the licensee failed to follow appropriate reporting requirements per Title 22 regulations. This poses an immediate health and safety risk to children in care.

(a) Each child receiving services from a family child care home shall have certain rights...These rights include, but are not limited to, the following: (2) To receive safe, healthful, and comfortable accommodations, furnishings, and equipment. This requirement is not met as evidenced by child not receiving medical attention after obtaining a head injury. Based on interviews conducted, the licensee failed to ensure that children in daycare are receive safe, healthful, and comfortable accommodations. This poses an immediate health and safety risk to children in care.
