Type B violation issued. Administrator was informed of an incident on 6/23/25 but failed to report it to the Department within the required timeframe.

Incomplete inspection due to time constraints. Critical safety violations in physical plant: unlocked door accessible to children with hazardous materials (water heater, paints, gardening tools), broken play equipment exposing sharp metal. Staff record discrepancies identified.

The case management inspection found the facility had not submitted an LIC 624 unusual incident report for a playground injury that occurred on July 2, 2024. The incident involved a child running into a black metal gate pole and sustaining a bump on the forehead, resulting in a Type B citation.

Complaint investigation substantiated that children are co-mingling across age groups, including school-age and preschool children being placed in infant room to calm down or during staff shortages. Type A citation issued.

Preschool center inspection found the classrooms, outdoor area, kitchen, and restrooms clean and well maintained, with lead testing completed and most records in place. The deficiencies were document-based: child 2 was missing a completed physician report, and mandated reporter training certificates were missing for all four staff present.

The annual inspection found a preschool classroom out of ratio by one child and staff members who were not associated with the preschool component present on site. The center otherwise had clean kitchens, safe outdoor space, current children's records, and an available disaster drill log.
