What Happened
The case management inspection found the facility had not submitted an LIC 624 unusual incident report for a playground injury that occurred on July 2, 2024. The incident involved a child running into a black metal gate pole and sustaining a bump on the forehead, resulting in a Type B citation.

This requirement is not met as evidenced by no LIC 624 submitted to the Department. Based on interviews conducted, the licensee failed to follow appropriate reporting requirements per Title 22 regulations. This poses an immediate health and safety risk to children in care.
