Based on interview and record review, the licensee did not comply with the section cited above. During today's inspection, an assistant did not have his TB test in file. LPA asked the director to follow up with the front office (Ave J) to see if the assistant had submit his TB test result. The office was unable to find it, which poses/posed a potential health, safety or personal rights risk to persons in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met by evidenced by :LPAs obsevation. LPA's obsevation. LPA's observed paint chip in the school age room on multipal areas in the room and at the enterance by the front desk. Based on LPA's observation, facility did not ensure a good environment repair which poses a potential risk to the health and safety of children in care.
