What the inspector wrote

Licensing Program Analyst (LPA) Keyona Scott conducted an unannounced Plan of Correction (POC) inspection on 01/31/2025 at 8:46 AM. The purpose of the inspection was to ensure the Type A deficiency cited on 01/21/2025 has been cleared and verify that the Licensee is operating within the limitations of the license. Upon LPA's arrival to the child care center office, there was no staff present in the office. LPA met with Assistant Director (A1) while walking to the infant classroom. LPA explained to Assistant Director (A1) the reason for the inspection. LPA continued to infant classroom and conducted tour. At 9:10 AM, LPA observed six (6) infants in the infant classroom transitioning from the breakfast table and changing station to the activity room, with four (4) staff, in which one staff (S1) was hired from a staffing agency. All adults present, working and or volunteering at the facility have a criminal record clearance or exemption. LPA attempted to review files to verify that Acknowledgement of Receipt of Licensing Reports (LIC 9224) were signed by each child's parent/authorized representative and was placed in each child's file; however, Assistant Director advised was still working on obtaining LIC 9224 signed by parent/authorized representative. Assistant Director advised that LIC 9224 and report was provided to parent/authorized representative however, parents/representative wanted to read report prior to signing LIC 9224. LPA advised Assistant Director that purpose of LIC 9224 is to have parent/representative confirm received a copy of the report and to have parent/representative sign LIC 9224 at time of receiving 3 - Year Required licensing inspection report dated 01/21/2025. Assistant requested additional time to obtain signed LIC 9224 from parent/authorized representative. LPA advised Assistant Director to send LPA copies of signed LIC 9224 along with child care roster for infant program no later than 02/04/2025. During today's inspection, LPA observed the following: All adults present, working and/or volunteering in the infant program have a criminal record or exemption. Teacher to infant ratios were adhered to. PAGE 1 During today's inspection, LPA observed the following (continued): Loose sheets on eleven (11) out of thirteen (13) cribs in the napping area at 9:15 AM; however, LPA did not observe any infants napping. LPA advised Assistant Director that infant crib mattress must have fitted sheets. Assistant Director advised fitted sheets purchased and will be placed on mattress cribs prior to infants napping. Licensee, Natasha Madison arrived at the facility with fitted sheets at 11:00 AM. LPA observed infants eating lunch at infant table at 11:07 AM. LPA observed infant mattresses being cleaned and fitted sheets being placed on infant mattresses at 11:10 AM. During today's inspection, LPA obtained the following documents: Declaration statement on how plans to ensure all persons present, working, or volunteering at facility have a criminal record clearance and are associated to the facility. Declaration statement referencing videos watched regarding adhering to teacher to infant ratios and plan on how to adhere to teacher to infant ratios. LPA will clear two (2) Type A deficiencies once signed LIC 9224 and infant program child care roster are received, no later than 02/04/2025. LPA, Keyona Scott, informed Assistant Director (A1), that report dated, 01/21/2025, documents two (2) Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA, Keyona Scott, informed Assistant Director (A1) to provide a copy of the licensing report dated 01/21/2025 that documents Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of the report dated 01/21/2025. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. PAGE 2 Assistant Director request additional time to complete plan of corrections for two Type B deficiencies cited during 3 - Year Required Annual inspection on 01/21/2025. LPA advised Assistant Director to complete plan of corrections for two (2) Type B deficiencies cited 01/21/2025, which includes conducting staff trainings for each deficiency and submitting meeting agendas and attendance sheet signed by all staff in attendance for each training to LPA no later than 02/07/2025. LPA will clear two (2) Type B deficiencies once plan of corrections are completed and submitted to LPA, no later than 02/07/2025. The facility was operating in substantial compliance during today\342\200\231s inspection on 01/31/2025. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies were cited. The following was thoroughly discussed: Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with th e Assistant Director. PAGE 3
