What Happened
Type A deficiency cited for uncleared adult staff member. Outdoor play area safety concerns noted with exposed nails and hazardous fencing. Carbon monoxide detector not operational and lead testing deadline not met.
Observations
- One Type A deficiency: Not all center staff had fingerprint clearance or were associated with designated license number
- Outdoor play area hazards: uneven ground, rough wood railings with exposed nails in process of being repaired
- Fencing hazard: children tall enough to reach wire
- Carbon monoxide detector not in service
- CPR card for staff member expired on 7/2023
- Lead testing not completed prior to deadline
