This requirement was not met as evidenced by: Based on observation and interview, LPA observed one toilet inside the boy's restroom not working properly and Staff disclosed the staff restroom was clogged as of 4/13/2025, and it was not reported to Licensing which is a potential health, safety, or personal rights risk to the persons in care.
