What the inspector wrote

On April 15, 2025, Licensing Program Analyst (LPA), Calloway conducted an unannounced case management inspection to the above facility. LPA met with the facility representative who granted access. LPA toured the facility with the representative and observed forty-one (41) school aged day care children and six staff in care. During the inspection, LPA observed one toilet inside the boy\342\200\231s bathroom did not work and flush properly. LPA asked to observe the staff restroom and per representative, the staff restroom was clogged on 4/13/2025, and the staff have been using the children\342\200\231s restrooms. Per representative, the plumbing issue was not reported to Licensing because they were not aware it needed to be reported. There is one deficiency cited during this inspection. See LIC 809D page attached to this report. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Aubree Marcelline, Representative, at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.

(4) All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition. This requirement was not met as evidenced by: Based on observation and interview, LPA observed one toilet inside the boy's restroom not working properly and Staff disclosed the staff restroom was clogged as of 4/13/2025, and it was not reported to Licensing which is a potential health, safety, or personal rights risk to the persons in care.
