What the inspector wrote

Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced annual inspection on 06/11/21. LPA met with Staff #4, Teacher, who guided analyst on a tour of the facility. This is an infant program which consists of 1 classroom; Infant Room. Facility operation hours are Monday to Friday from 6:00 AM to 6:00 PM. Director Sandy Mc Glothan arrived toward the end of the tour. The infant program is located adjacent to the center's pre-school program #197493731 and the school-age program #197493729. The infant program has no shared areas. All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room 1: Staff #4 with no children present. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and good repair. LPA observed a baby walker and a baby bouncer in the classroom. LPA advised Teacher that baby walkers and bouncers are not allowed on the premises of any child care facility because they pose a risk to the health and safety of children in care. Staff removed the items during the inspection. The facility has sufficient napping equipment, feeding chairs, and changing tables. Cribs were observed to be free from loose articles and objects including blankets, pillows and no objects were observed to be hanging above or to the side of the cribs. The facility has a separate napping area. LPA advised that no infants are to be swaddled. LPA provided the licensee with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics and consulted about Never Shake a Baby, and Safe Sleeping practices. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are taken home each week to be washed. The isolation area is located in the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classroom.---Page 1 of 3 Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Poisons were observed to be stored in the the kitchen in a locked cabinet. LPA advised to keep the kitchen locked with key lock or to store the poison under key or padlock. Carbon monoxide detectors were observed and are operable. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Menu was observed to be posted in the kitchen. LPA advised that the menu be posted in an area in which parents can see. Food in refrigerator was labeled. Infant formula is available. Kitchen counter tops were clean and free of clutter or standing food. The outdoor playground was observed to be physically separate from the preschool and school-age components. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility. Staff present has obtained a criminal record clearance or criminal record exemption. Staff present did not have training in CPR and Pediatric First Aid present during this inspection. Children’s Records were reviewed. Infant Needs and Service plans were reviewed and observed to be in each file. However, LPA did not observe the LIC 9227 Individual Infant Sleeping Plan for the four infants enrolled under 12 months. LPA advised staff that infants need to be physically checked when sleeping every 15 minutes and document whether there is labored breathing, signs of distress, and sleep position (on their backs). The name, address, telephone of child’s authorized representative, and medical assessment where on file. Inspection of required forms was made and documented on the LIC 857. ----Page 2 of 3 LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present did not have proof of renewal of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. First Aid supplies were observed in the classroom on a shelf. According to Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the office. Per Director, there are no children currently on medication. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA provided the Director with a copy of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD a nd Safe Sleep Awareness: PIN 19-02-CCP, COVID FAQ The following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.     Personnel Requirements:(f) At least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid pursuant to Health and Safety Code Section 1596.866 shall be present when children are at the child care center or offsite for center activities. This requirement is not met as evidenced by LPA observing that Staff present does not have Pediatric 1st Aid and CPR. This poses a potential health and safety risk to children in care.

This poses a potential health and safety risk to children in care.

    Infant Care Center Fixtures, Furniture, Equipment and Supplies(d)(2) A baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Section 1596.846. This requirement is not met as evidenced by LPA observing a baby walker and baby bouncer at the facility. There were no infants at the time of observation therefore, this poses a potential health and safety risk to children in care.
