    Infant Needs and Services Plan(b)(2) Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan [LIC 9227 (3/20) This requirment is not met as evidenced by the facility not having the LIC 9227 for infants up to the age of 12 months on file. This poses a potential health and safety risk to children in care.
