What the inspector wrote

On 01/15/2026 LPA Clayton conducted an unannounced Case Management inspection at Kids Kingdom. LPA was greeted by Director Coreen Salceda. LPA Clayton toured the CCC inside and outside for Health & Safety inspection and observed 5 infants being supervised and cared for by 2 staff. LPA Clayton reviewed Guardian and observed teacher S1’s fingerprint submission determination status "in process" since June 2025, which poses an immediate Health and Safety risk to children in care. LPA Clayton observed a “baby walker” in classroom 1, and a bouncy seat in the infant nap room which poses an immediate Health and Safety risk to children in care. LPA Clayton reviewed the infants files, none of which contained the Individual Infant Sleep Plan (LIC 9227), or the required Infants Needs and Services plan. In addition, it was determined that the staff have not documented checking on the sleeping infants every 15 minutes as required per Title 22 regulations. Based on LPA observations and record review, two (2) Type A violation and two (2) Type B Violations were cited today (see LIC 809D’s). Licensee will provide parents of currently enrolled children and parents of children enrolled over the next 12 months copies of the Licensing Report containing the Type A violation and maintain a signed LIC 9224 the above-mentioned children’s files. An exit interview was conducted, and this report and Appeal Rights were discussed and provided to Director Coreen and Licensee Trina Mendoza. LPA Clayton posted a notice of site visit which must remain posted for 30 days.

This requirement was not met as evidenced by: LPA Clayton's review of Guardian and observed staff fingerprint submission determination status "in process" since June 2025, which poses an immediate Health and Safety risk to children in care.

(1) A baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Sections 1596.846(b) and (c). (b) A baby walker shall not be kept or used... facility.(c) A "baby walker" means any article described in paragraph (4) of subdivision (a) of Section 1500.86 of Part 1500 of Title 16 of the Code of Federal Regulations. This requirement was not met as evidenced by: LPA Clayton observation of a baby walker and a bouncy seat observed on the premises, which poses an immediate Health and Safety risk to children in care.

    (a) Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant. (b) The needs and services plan......include the following: (1)The individual feeding plan. (2) Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan [LIC 9227 (3/20)], which is incorporated by reference. The requirement was not met as evidenced by: LPA Clayton's review of the 6 infants files, and Licensees and Director's admission that they have not created the Infant needs and Services Plans and no Individual Infant Sleeping plans were present which posed a Health and Safety risk to children in care.

    (2) Sleeping infant(s) shall be directly observed by sight and sound at all times.(B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following: 1. Labored breathing. 2. Signs of distress, which includes but is not limited to flushed skin color, increase in body temperature and restlessness. 3.Infants up to 12 months of age who are sleeping in a position other than on their back. a. If the infant’s Individual Infant Sleeping Plan [LIC 9227 (3/20)] does not have Section C completed, staff shall return the infant to their back for sleeping. 4. If staff observes labored breathing or signs of distress.......(C) Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following:1. Date.2. Infant’s name.3. Time of each 15-minute check.4. Initials of staff person whoconducted each check. The requirement was not met as evidenced by: LPA Clayton's review of the 6 infants files, and Licensees and Director's admission that they have been using a Sleep Log to document checking on the infants every 15 minutes as stated in the regulation, which poses a Health and Safety risk to children in care.
