Documentation shall include the following:1. Date.2. Infant’s name.3. Time of each 15-minute check.4. Initials of staff person whoconducted each check. The requirement was not met as evidenced by: LPA Clayton's review of the 6 infants files, and Licensees and Director's admission that they have been using a Sleep Log to document checking on the infants every 15 minutes as stated in the regulation, which poses a Health and Safety risk to children in care.

The requirement was not met as evidenced by: LPA Clayton's review of the 6 infants files, and Licensees and Director's admission that they have not created the Infant needs and Services Plans and no Individual Infant Sleeping plans were present which posed a Health and Safety risk to children in care.
