What Happened
Incident investigation determined lack of care and supervision occurred when child left unattended in classroom during outdoor transition. Staff failed to follow proper tracking procedures, resulting in child being found asleep in classroom after approximately 3 minutes.
Observations
- Staff 1 did not follow proper transition tracking procedures leaving Child 1 unattended in classroom
- Child 1 left sleeping inside classroom during transition from 7/8 classroom to preschool yard
- Approximately 3 minutes elapsed before name-to-face roll call discovered child missing from group
- Child found by staff sleeping inside classroom

Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not being met as evidenced by: On 10/31/24, at 10:55AM, during transition from the 7/8 classroom to the preschool yard, Staff 1 did not follow proper transition tracking procedures leaving Child 1 sleeping inside the classroom. This poses an immediate health, safety, and personal rights risk to children in care.
