Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1), a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) shall be submitted to the Department within seven days following the occurrence of such event. This requirement is not being met as evidenced by: On 7/22/25, LPA Katrdzhyan discovered that an alleged incident involving Staff 1 was not reported to Community Care Licensing (CCL), as required by Title 22 Regulations. The alleged incident occurred on 7/11/25, and involved a report of possible misconduct by Staff 1. Based on the nature of the alleged incident, it should have been reported to CCL in accordance with Title 22 Regulations Section 101212 / Reporting Requirements. This poses a potential health, safety or personal rights risk to persons in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not being met as evidenced by: On 10/31/24, at 10:55AM, during transition from the 7/8 classroom to the preschool yard, Staff 1 did not follow proper transition tracking procedures leaving Child 1 sleeping inside the classroom. This poses an immediate health, safety, and personal rights risk to children in care.

California Lead Action Level at Child Care Centers

California Lead Action Level at Child Care Centers. A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance.This requirement is not met as evidenced by: Lead water testing samples taken on 11/11/2022 revealed that levels of lead were beyond the allowable amount deemed by the State of California. Water Outlet “N” has Action Lead Exceedance (ALE) “N” 8.36ppb. This is a type B violation which poses a potential Health, Safety or Personal Rights risk to persons in care.
