What the inspector wrote

On 8/18/25, at 10:30AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced follow up Case Management Visit to this facility. Upon arrival, LPA met with Director / Christina Lechner, who guided LPA on a tour of the facility. There were 50 children with 14 staff observed present in the preschool program. LPA explained the purpose of today's visit is to follow-up on an incident that was reported to Community Care Licensing (CCL) on 7/23/25. The alleged incident which occurred on 7/11/25, was reported to the Department on 7/23/25, via Unusual Incident/Injury Report. During the course of the investigation, interviews were conducted, and copies of children's and staff rosters, along with other supporting documents, were obtained and reviewed. Information reported to the Department indicated that possible misconduct may or may not have occurred in the incident involving Staff 1. During the course of the investigation, LPA Joe Katrdzhyan and Special Investigator Douglas Real conducted interviews related to the alleged incident. Based on the findings, there is insufficient evidence to determine whether possible misconduct was a factor in the incident involving Staff 1. Therefore, the allegation is deemed unsubstantiated. No additional follow up is necessary regarding the incident reported. There were no deficiencies cited during today’s inspection. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. An exit interview was conducted with Christina Lechner and Appeals Rights provided.

Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1), a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) shall be submitted to the Department within seven days following the occurrence of such event. This requirement is not being met as evidenced by: On 7/22/25, LPA Katrdzhyan discovered that an alleged incident involving Staff 1 was not reported to Community Care Licensing (CCL), as required by Title 22 Regulations. The alleged incident occurred on 7/11/25, and involved a report of possible misconduct by Staff 1. Based on the nature of the alleged incident, it should have been reported to CCL in accordance with Title 22 Regulations Section 101212 / Reporting Requirements. This poses a potential health, safety or personal rights risk to persons in care.
