What the inspector wrote

On 8/18/25, at 1:25PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. There were 50 children with 14 staff observed present in the preschool program. During a site visit conducted on 7/22/25, LPA Katrdzhyan discovered that an alleged incident involving Staff 1 was not reported to Community Care Licensing (CCL), as required by Title 22 Regulations. The alleged incident occurred on 7/11/25, and involved a report of possible misconduct by Staff 1. Based on the nature of the alleged incident, it should have been reported to CCL in accordance with Title 22 Regulations Section 101212 / Reporting Requirements. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Christina Lechner, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

In addition, a written report containing the information specified in (d)(2) shall be submitted to the Department within seven days following the occurrence of such event. This requirement is not being met as evidenced by: On 7/22/25, LPA Katrdzhyan discovered that an alleged incident involving Staff 1 was not reported to Community Care Licensing (CCL), as required by Title 22 Regulations. The alleged incident occurred on 7/11/25, and involved a report of possible misconduct by Staff 1. Based on the nature of the alleged incident, it should have been reported to CCL in accordance with Title 22 Regulations Section 101212 / Reporting Requirements. This poses a potential health, safety or personal rights risk to persons in care.
