**THIS IS AN AMENDED REPORT WHICH SUPERSEDES THE PREVIOUS REPORT** On 08/13/2025 at 01:35 PM, Licensing Program Analyst (LPA) Elicia Calvillo and Licensing Program Manager (LPM) Rita Ramos conducted an unannounced Case Management-Other visit. LPA and LPM identified self and met with Christina Lechner, Director who guided LPA and LPM on a tour of the inside and outside of the facility. Upon arrival there were 53 children and 11 staff. The purpose of today’s visit is to obtain a copy of documentation regarding an incident that occurred on 07/11/2025 During today's visit, no citations were issued. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. The Exit interview was conducted with Christina Lechner, Director including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
