LPA made observations and observed the inside and outside of the facility is in need of repair due to chipped paint inside and outside exposing metal frame and old paint underneath, asphalt in the outdoor playground is buckled and poses a hazard to children, unused licensed classrooms and restroom are not set up for use. There are broken toys in the outdoor space that need to be removed or repaired. Wooden gate surrounding facility needs to be painted and broken pieces need to be repaired. All petal and wooden posts need to be covered with a soft cushioning material. Based on the information obtained through observations and interviews, the allegation is substantiated. A substantiated finding means that the complaint is substantiated and the allegation is valid because the preponderance of the evidence standard has been met. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, Jacqueline Soria Lopez.
