What the inspector wrote

On 02/06/2025, Devon Carus & Adrian Risher Licensing Program Analysts (LPAs) conducted a subsequent visit to deliver the findings for an Unusual Incident Report (UIR) reported to the department by the licensee. A full investigation was conducted by Investigator Veronica Padilla of the Department\342\200\231s Investigation Bureau regarding a child that was injured at the facility. The incident occurred at a prior licensed family childcare home licensed by the same provider(1974 94837). However, since the incident the licensee relocated to where she is currently providing care. Based on the evidence obtained during the investigation, which included interviews with relevant parties, relevant documentation obtained to include medical records there is a preponderance of the evidence, to show that the allegation occurred, therefore the above allegation is substantiated. A child in care sustained a fractured leg when the licensee did not use care on placing a child in a crib for a nap. Based on the child\342\200\231s age the crib was not age-appropriate which is a personal rights violation and led to lack of supervision by the licensee when she did not check on the child timely to ensure child had not sustained any injuries and was free from pain. This is an immediate risk to the health & safety risk for children in care and a Type A violation. A $500.00 civil penalty will be assessed for lack of timely care & supervision. The civil penalty is being assessed on the Harbor City address for the record. This location is where the incident occurred. Upon receipt of this report, the Licensee shall post the Notice of Site Visit (LIC 9213) and any Licensing report documenting a type \342\200\234A\342\200\235 deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224) form must be maintained in each child\342\200\231s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224). Exit interview was conducted and a copy of the report was provided. Appeal rights were reviewed and provided.
