What the inspector wrote

On 4/22/25, at 1PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. There were 10 children with 5 staff observed in the infant program and 7 children and 2 staff observed in the toddler program. During a site visit conducted on 4/16/25, LPA discovered that on 2/4/25, facility staff had been combining children from the toddler program in Room CR1 with children from the infant program in Room CR2. Per Health and Safety Code 1596.956 / Child day care centers serving infants; optional toddler program; departmental guidelines and regulations, the toddler program shall be conducted in areas separate from those used by older or younger children. This poses a potential health, safety or personal rights risk to persons in care. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Lorena Castro, Interim Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement is not being met as evidenced by: The incident occurred in January of 2025 (exact date unknown), during which time Staff 2 called out and Staff 1 was left alone to take care of eight infants. It reportedly took more than 10 minutes before an additional staff member was located and assigned to assist Staff 1. Failure to report the incident to CCL as required, poses a potential health, safety or personal rights risk to persons in care.
