Child day care centers serving preschool age children; optional program; departmental guidelines and regulations.A ratio of six children to each teacher is maintained for all children in attendance at the toddler program. This requirement was not being met as evidenced by: On 6/16/25, at 8:50AM, Child 1 was dropped off by their parent in Room 1 (Toddler Program). At the time of drop-off, there was only one teacher present (Staff 1) in the classroom, supervising six children. With the addition of Child 1, the total number of children increased to seven, placing the classroom out of compliance with the required teacher-to-child ratio for the toddler age group. Staff 2 arrived at 8:52AM, at which point the classroom returned to compliance. Room 1 was out of ratio for approximately two minutes.

All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 101170(f). This requirement was not met as evidenced by: During a cite visit conducted at the facility on 06/19/25, LPA discovered that Staff / Edith Cristina Henriquez Sosa was working at the facility prior to the Licensee requesting a transfer of a criminal record clearance as specified in Section 101170(e)(2). This poses a potential health, safety or personal rights risk to persons in care.

The toddler program shall be conducted in areas separate from those used by older or younger children. Plans to alternate use of outdoor play space may be approved to achieve separation. This requirement was not being met as evidenced by: During a site visit conducted on 4/16/25, LPA discovered that on 2/4/25, facility staff had been combining children from the toddler program in Room CR1 with children from the infant program in Room CR2. This poses a potential health, safety or personal rights risk to persons in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.This requirement is not met as evidenced by: During the visit conducted on 10/31/24 by LPA Katrdzhyan, infestation of ants were observed in the infant classroom top/upper cabinet, where snacks are stored for children in care. Licensee was asked to schedule a licensed pest control company to inspect/treat the ant infestation and provide a copy of the invoice to CCL. The POC was not met. This poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above. Part of the toddler classroom wall located outside, in the toddler play area was in disrepair. There was a large hole in the wall with mesh wires and pipes visible on the inside. The hole was being covered by a cardboard box with sand equipment/toys in front. This poses a potential health, safety or personal rights risk to persons in care.
