3-year required inspection of child care center licensed for 37 children (30 preschool, 7 toddlers). Four Type B deficiencies cited: staff supervision gap observed, missing infant sleep charts, incomplete mandated reporter training documentation, and lead testing not yet completed.
19-month-old toddler found napping in preschool room with 6 preschool children instead of toddler classroom, creating supervision and age-separation violation. Type A citation issued.
The complaint inspection included interviews with the director and staff plus review of staff and children files. LPA also requested the children's roster, sign-in sheet, and parent contract before substantiating the allegations and issuing citations.
