What the inspector wrote

On 11/4/2025, Licensing Program Analyst (LPA) Doris Whitmore conducted a unannounced case management inspection to follow up on an Unusual Incident, reported to the department by email on 10/31/2025. LPA toured the facility and took a census of how many children and staff are present today. LPA observed 3 total children present with 3 staff. Members. LPA met with Trymaine Bryant Floater Teacher and spoke with Center Director Chasti Neal on the phone. According to the LIC 624 Form, Unusual Incident Report Form, (C1) went home after bumps were observed on her hands, feet, and inside of her mouth. Mom confirmed with Fani the doctors diagnosed it as HFM. On 10/29/2025 the mother of (C2) called to notify Fani he had bumps on his feet. Mom informed Fani on 10/30/2025 child was seen by the doctor and diagnose with HFM. On 10/31/2025 the mother of (C3) informed Fani that child went to the doctor as was diagnosed with HFM. During the inspection, LPA toured the facility and observed that (C1) and (C2) were present today and have been cleared to return to school. (C3) was absent today. A deep Cleaning was done over the weekend by Continental Cleaning Company. Teachers sanitized the toys, and all washable items were washed. Parents were notified through the Family Advocate. Printed letters and what to look for. LPA was able to review the handouts that were given to the parents. LPA reviewed the doctor\342\200\231s notes for all three of the children. LPA concluded this case management inspection visit, No Title 22 violations have occurred, and no deficiencies cited. A notice of site visit was given and posted for 30 days. Exit interview conducted and report was reviewed with Floater Teacher, Trymaine Bryant.
