What the inspector wrote

On 12/12/2025, LPA Clayton conducted an unannounced visit to the Light of Knowledge Child Care Center to deliver the findings on a complaint. LPA Clayton was greeted by teachers Salwa Awad. Director Rafeda Shamsa arrived later. LPA Clayton toured the facility for a Health and Safety inspection and observed 8 preschool children in care, being cared for and supervised by 1 fingerprint cleared staff. On 10/08/2025 ESCCRO received a complaint alleging the following: • PERSONAL RIGHTS: Child sustained unexplained injury while in care. • FOOD SERVICE: Staff not meeting daycare child’s dietary needs resulting in allergic reaction. On 10/09/2025 LPA Clayton toured the CCC inside and outside for a Health and Safety Inspection, reviewed staff and children’s files and interviewed staff. LPA Clayton conducted a thorough investigation which included facility inspections, observations, and staff interviews referencing the above-mentioned allegations. LPA Clayton LPA Clayton observed teacher S1 become overwhelmed and pull several students by the arm and wrist. LPA interview with Director who acknowledged that on at least one occasion, the child mentioned in the complaint was served corn and it is indicated in the child’s file that she has a corn allergy.

    101223 (a) (1) (3) Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, …….or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting……. This requirement was not met as evidenced by: LPA Clayton's observation of the staff forcing the children to sleep and forcing the children to wake up from their nap which poses a Health and Safety Risk to children in care.

    101230 Activities (a) Each center shall provide a variety of daily activities designed to meet the needs of children in care, including but not limited to: (4) Toileting. This requirement was not met as evidenced by: LPA Clayton's observation that the children in the Dolphin class are not having their toileting/diapering needs addressed until a another teacher arrives at 10am, which posed a Health and Safety risk to children in care.

This requirement was not met as evidenced by: The child in care suffered a dislocated elbow in care, and LPA Clayton's observation of teacher S1 pulling children by the wrist, and arm, which poses and immediate Health and Safety risk to children in care.

    (7) Modified diets prescribed by a child's physician as a medical necessity shall be provided. (B) A child shall not be served any food to which the child's record indicates he/she has an allergy. This requirement was not met as evidenced by: Director's acknowledgement that on at least one occsasion, the child in care was served food indicated in her file as an allergen, which posed an immediate Health and Safety risk to children in care.
