What the inspector wrote

On 5/1/26, Licensing Program Analyst (LPA) V. Wheatley conducted a case management inspection. The director Rafeda Shamsa was present with Staff #1 and Staff #2. Upon arrival, LPA Wheatley observed 12 children on the premises inside of the enclosed classroom napping. LPA observed the children supervised by Staff #1. The purpose of the inspection is to cite the facility for a previous violation. LPA was informed that a child was injured on the premises on July 11, 2025. The director failed to report the incident to the Department whereby the child was injured with scissors. The child was administered first aid and was supervised during the time by a staff member. The parent was called and the child was taken to the doctor. Based on LPA's observations, this is a Type B violation for failure to report the incident. (See LIC 809D report.) In addition, the facility is being assessed $500 civil penalty for lack of supervision based on the last report dated 4/24/26. (See LIC 421M form) An exit interview was conducted. A copy of this report was read and provided to the director. A Notice of Site Visit will be posted for 30 days.

    101212 (d)(1)(B)-Reporting Requirements- Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. This requirement was not met as evidenced by: LPA Wheatley was informed by the director and other witness that Child #1 was injured with scissors while at the day care. The child was taken to the doctor. The injury was not reported to Licensing. This poses a potential risk to the immediate Health and Safety risk to the health and safety child in care.
