What the inspector wrote

On 2/6/2025, at 1:06pm Licensing Program Analysts (LPA)s Devon Carus, and Adrian Risher conducted a case management inspection to examine/ observe the additions the licensee plans to add to the outdoor childcare area. LPAs toured the area and observed children's toys, walls high enough to keep children from climbing out of the area, a swing for children, and properly installed turf grass. During the inspection, LPAs Carus and Risher observed some electrical wires that come from the corner of the house leading to the roof. LPAs observed that these wires are essential regarding use of electricity in the family child care home, but are within reach of children. Licensee agreed to set up and secure a children's safety gate around this area, and send photo evidence to the email of LPA Carus that will require the department's approval. LPAs Carus and Risher also observed a children's swing in the proposed childcare area. Licensee was advised to properly store this swing out of reach of children during daycare hours, and will send photo evidence to the email of LPA Carus that will require the department's approval. Due to the nature of the proposed space, licensee will ensure that she maintains 100% visual supervision of children in care at all times. Licensee also agrees to send an updated outdoor facility sketch (LIC 999) to the email of LPA Carus by 12/10/2025 that will require the department's approval. Licensee will provide proof of completion of these three items, and this outside play area will remain inaccessible until the licensee receives the department's approval. Exit interview conducted. A notice of site visit was provided to the licensee, and must remain posted for 30 days.

This requirement was not met as evidenced by: A child sustained a fractured leg when the licensee did not carefully place the child in a crib. Licensee did not check on child timely to render aid. Licensee did not utilize age-appropriate napping equipment for the child. This is an immeditate health & safety risk for children in care.

This requirement was not as evidenced by: A child sustained a fractured leg when licensee did not use care on placing a child in a crib for a nap.Based on the child’s age the crib was not age-appropriate which is a personal rights violation and led to lack of supervision when licensee did not check on child timely to ensure child was free from pain. This is a health & safety risk for children in care.
