What the inspector wrote

This is a duplicate report that is being amended due to it being entered on this facility incorrectly. On 2/6/2025, Devon Carus & Adrian Risher Licensing Program Analysts (LPAs) conducted a subsequent visit to deliver the findings for an Unusual Incident Report (UIR) reported to the department by the licensee. LPAs observed 4 children in care at the time of the inspection. The incident occurred at a prior licensed family childcare home licensed by the same provider (197494837). However, since the incident the licensee relocated to where she is currently providing care. No citations for this facility were issued today. Exit interview was conducted and a copy of the report was provided.

This requirement was not met as evidenced by: A child sustained a fractured leg when the licensee did not carefully place the child in a crib. Licensee did not check on child timely to render aid. Licensee did not utilize age-appropriate napping equipment for the child. This is an immeditate health & safety risk for children in care.

    1024117 (a) Operation of a Family Childcare Home (a) The licensee shall be present in the home and shall ensure that children in care are supervised at all times…. This requirement was not as evidenced by: A child sustained a fractured leg when licensee did not use care on placing a child in a crib for a nap.Based on the child’s age the crib was not age-appropriate which is a personal rights violation and led to lack of supervision when licensee did not check on child timely to ensure child was free from pain. This is a health & safety risk for children in care.
