On 5/1/2025 at 1:30pm, Licensing Program Analyst (LPA) Devon Carus conducted a case management visit to amend a previous report. LPA met with Mayura Vhanakaware, Licensee and explained the purpose of the visit. LPA observed 5 children in care. The purpose of the visit is to amend the Case Management - Incident report from 2/6/2025. LPA observed the facility to be clean, and orderly. Licensee will continue to provide care for the following ages: 12 weeks to 14 years of age. Licensee confirmed her operating hours are 8am - 5:30pm, Monday through Friday. Additionally, LPA Carus also inspected the indoor child care rooms, child care restroom, back, and side yard areas, and observed them to be safe and free of hazards. Based on observations made by the LPA, no deficiencies will be cited today . LPA did not observe any violations during today's visit. Facility will continue to be under increased monitoring on a quarterly basis. Exit interview was completed with Mayura Vhanakaware, Licensee. A notice of site visit was provided.
