What the inspector wrote

On 12/5/2025 at 9:00 am, Licensing Program Analyst (LPA) Devon Carus conducted a case management visit for increased monitoring. LPA met with Mayura Vhanakaware, Licensee and explained the purpose of the visit. LPA observed 4 children in care. The purpose of the visit is to ensure the Licensee is maintaining a safe and comfortable environment for the children in care. LPA observed the facility to be clean, and orderly. Licensee will continue to provide care for the following ages: 12 weeks to 14 years of age. Licensee confirmed her operating hours are 8am - 5:30pm, Monday through Friday. LPA Carus reviewed the enrolled children's files, and observed the LIC 9224 forms regarding the recent Type-A violation, and Non-Compliance Conference signed by each parent. Additionally, LPA Carus requested a copy of the facility roster, and observed there to be no new children who have enrolled since the previous visit on 08/28/2025. LPA Carus also inspected the entirety of the child care, and observed it to be safe and free of hazards. Based on observations made by the LPA, no deficiencies will be cited today . LPA did not observe any violations during today's visit. Facility will continue to be under increased monitoring on a quarterly basis. Exit interview was completed with Mayura Vhanakaware, Licensee. A notice of site visit was printed, issued, and must remain posted for 30 days.
