What the inspector wrote

On 08/27/2025 , Licensing Program Analyst (LPA) Cristina Castellanos conducted an announced Pre-licensing Inspection. The purpose of this inspection is to ensure that health, safety and personal rights as required by Title 22 Regulations governing California Child Care Homes will be met by the applicant. Upon arrival LPA introduced herself and disclosed the purpose of the inspection. LPA was greeted and granted entry into the home by Applicant, Miaoxin Wen. Present during today\342\200\231s inspection was only Applicant Wen. The applicant is applying for a Large Family Child Care Home with a max capacity of 14 as well as a Change of Location. Control of property was submitted to the El Segundo Child Care Regional Office for review. On 05/27/2025 a fire safety inspection was completed, and a fire clearance was granted by the Redondo Beach Fire Department. Per the application submitted, applicant Wen wishes to provide services to a 1.5-month-old to 6 years old between the hours of 08:30 a.m. to 5:30 p.m., Monday - Friday. Per facility sketch and tour of the home, the following was observed and discussed with the Applicant during the inspection: The home is part of a multi-family two story duplex. The applicants\342\200\231 unit is located in front of the premises, and it consists of two levels. The 1 st floor has a living room, a half bathroom, a kitchen area, a backyard and a detached two-door garage located at the back of the premises. The 2 nd floor has 3 bedrooms and one bathroom. Continue The applicant confirmed that the areas that are inaccessible (OFF LIMITS) to children are the entire second level, the kitchen area, the front yard and the detached garage. LPA reminded applicant that any area designated as OFF LIMITS should be made inaccessible during the hours of operation and/or while children are present. Families will enter through the main door of the home where they will be led to the main living room. The living room was observed and inspected. Per applicant the living room will be the designated day care space. The living room was observed to have three (3) tables with chairs designated for children, two (2) storage units with plastic boxes, three-shelf open storage cabinets throughout the living room, a vertical building brick base, a puzzle maps cabinet set and a bookshelf. Applicant Wen confirmed children will nap and eat in the living room. Applicant confirmed the home will not be providing meals and/or snacks. Per applicant parents will ensure they provide meals and snacks for their children, which will comply with dietary restrictions and allergies. LPA discussed the importance of having procedures in place to work with allergies and/or dietary restriction. LPA encouraged applicant to contact their local Resource and Referral agency to inquire about any food programs in the area and any other resource. LPA observed and inspected all kitchen cabinets and counter space. LPA observed all cabinets to have child safety strap locks. All sharp objects and knives are stored away. All cleaning compounds were observed to be in the bottom kitchen sink cabinet with a child safety strap lock, making content inaccessible. The following was also observed by LPA during inspection: \302\247 All adults living in the home and assistant and/or substitute care providers have submitted fingerprints and child abuse index check forms to Department of Justice and received a California clearance or exemption. \302\247 Home is neat and clean. \302\247 Home has a brand new fully charged fire extinguisher, which is a 2A:10BC located between the side door exit and the children\342\200\231s designated bathroom. \302\247 Home has a smoke alarm located on the second floor and carbon monoxide detectors in both floors. \302\247 Home has a working telephone. \302\247 All poisons are locked. Page 2 \302\247 Hazardous materials are kept out of the reach of children (inaccessible): Kitchen: all sharp utensils and cutlery, cleaning supplies, medicines, drawers and cabinets with liquor, plastic bags, and sharp things or small things children can swallow, etc. Bathroom: shampoo, mouthwash, toothpaste, medicines, perfumes/lotions/cosmetics, solvents, etc. Garage and Outdoors: solvents, gasoline, oil, turpentine, paint, sharp tools, lawnmower, gardening tools, poisonous plants, abandoned machinery, old refrigerators/freezers, old vehicles, etc. \302\247 Per Applicant, there are no firearms or weapons in the home. None were observed by the LPA during inspection. \302\247 The outdoor play area is not fenced all around, currently it only has one extra wide retractable baby gate on one side of the home. \302\247 Home has a child safety gate blocking the bottom of the stairs leading to the 2 nd level. \302\247 Home does not have a swimming pool, spa, hot tub, fishpond, or any other bodies of water. \302\247 Toys and playthings are safe, clean, and appropriate for the age of the children. \302\247 Babywalkers, bouncers, jumpers, and similar items will not be used for children in care and are kept inaccessible. The following corrections need to be made before licensure: 1. The top of the staircase needs a safety gate. 2. Both the washer and dryer need to have a child safety strap lock or a cover to make them inaccessible to children in care. 3. The side walkway leading to the backyard needs a gate. 4. Applicant will complete form LIC855 - Declaration ensuring both programs will operate independently of each other, Additionally will include how parents will be made aware of 2 programs located on the same premises. 5. Applicant will submit an Outdoor Play Schedule. Corrections shall be submitted by 09/10/2025 . The licensing determination of this application will then be reviewed with Licensing Program Manager for final resolution. Page 3 Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Applicant rents/leases the home and has landlord consent: Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). Applicant knows prospective clients will need IMS: This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP. A Plan for Providing IMS must be submitted to the Department. Additionally, the LIC 126 - Entrance Checklist and Prohibited Items in a Licensed Child Care Home flyer were provided to the applicant. Safe Sleep LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Page 4 Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Megan\342\200\231s Law On this date, 05/16/2025, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. MyChildCarePlan.org Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Subscribe to CCLD important information Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
