What the inspector wrote

On February 25, 2026, Licensing Program Analyst (LPA) Annelise Villa met with Licensee Andreana Thomas, who guided LPA on a tour of the facility. The purpose of this visit was to conduct a Case Management - Incident inspection regarding an incident that occurred on January 26, 2026. This Unusual Incident was self-reported. LPA discussed reporting requirements, pursuant to California Code of Regulations (CCR) Title 22, Section 102416.2 to ensure all reportable unusual incidents are reported within specified time frames. Upon arrival LPA observed 0 children in care at the time inspection. Description of incident #1: On January 26, 2026, at approximately 04:00pm, C1s finger was pinched in the front door by the Licensee. The Licensee was closing the door after landscaper was leaving the premises. C1 was standing next to Licensee, looking out the window near the door. As Licensee closed the door, C1's finger was unknowingly near the hinges of the doorframe and the door pinched the tip of C1's left middle finger. C1 sustained a 2-3 centimeter red mark on the left middle finger. Licensee stated she applied cold running water and an ice pack to reduce pain and swelling. Parents were notified and incident report was given. The information obtained during the unusual incident follow up revealed no violations were found, however, Licensee stated she will continued ensure children are a safety distance from any hazards. Facility is encouraged to continue to report incidents that occur in the facility. An exit interview was conducted, a copy of this report was provided along with the notice of site visit.
